Narrative for Holy Family Hospital at Haverhill

At Holy Family Hospital, a Fall Prevention Protocol is in place. The protocol involves an on-going safety assessment for each patient which starts at admission and continues throughout the hospital stay. Appropriate interventions are put in place for patients at low, medium and high risk. We have a multidisciplinary Falls Task Force which reviews every fall. All staff members are educated on our Fall Prevention Protocol in orientation and annually thereafter. Additionally, our bed and chair alarms which are utilized for our patients identified as high risk have been updated. We continue to explore process improvement strategies to decrease patient falls, implement evidence base best practice and improve patient experience.

Our efforts to prevent pressure ulcers involve identifying patients at risk and implementing prevention strategies. Our Pressure Ulcer Prevention Protocol includes risk assessment on admission and daily through discharge via the use of the Braden Scale. We enhanced our program to include a root cause analysis for every pressure ulcer stage III or above and educating RNs on each inpatient unit with in-depth training on prevention and treatment of pressure ulcers. We promote the utilization of our Wound and Skin nurses to provide guidance and care planning for prevention, detection and treatment. These focused efforts allow us to implement best practices.
